Consent to treat an unaccompanied child
Should the parent or legal guardian of a child under 16 years old NOT be present when that child attends this dental practice for a booked appointment, sorry, we will not be able to treat them UNLESS the parent or legal guardian has previously informed the dental practice, i.e. given their consent.
The attached form below is to be completed by the parent or legal guardian and must be provided to the dental reception team prior to, latest at the same time of, the child's arrival at the dental practice.
